Teen Mentoring Program:
Helping Students become Self-Determined

Matt, Sean and Clayton
completed their "Hopes and Dreams" booklets.  It helped them express their strengths and abilities, and identify goals they want to reach in life,  including where they want to live and work.

Aaron completed his "Tree of Life" activity, which helped him think about who he is, what his strengths are, and what he enjoys doing. After completing the activity, he shared it with his his peers, which enabled him and the others to see their similarities and differences.

Alexis 
enjoyed talking about her strengths and weaknesses and how to make each weakness something positive.  This activity  is called "Pieces of Life."

Randy and Suzie completed the "Healthy Me" activity,   which helps students recognize the importance of keeping a healthy mind and body.  

​ "Healthy Me" targets 4 key areas:

1. Sleep
2. Personal Hygenie
3. Physical Activity
4. Nutrition

PATH Program in action!

Clayton completed his PATH (Planning Alternative Tomorrows with Hope) and is now looking for work to make money and get experience.  Two goals Clayton identified in his PATH were to get a job and make money.  One of his dreams is to go to a Packer's game!

MAP Program in action!

Ian completed his MAP (Making an Action Plan), which helps students identify their
​interests and abilities and set goals for the future.
